Chad Johnson headbutted ex-wife Evelyn Lozada so hard during their infamous domestic dispute last year, she was left with a massive bloody gash on her forehead — and TMZ has obtained photographic evidence.

The photos were taken by Davie Police in the Florida emergency room where Evelyn was transported following Chad’s attack on August 11th, 2012. According to police, the forehead laceration measured 3 inches.

TMZ broke the story … Johnson was arrested for domestic violence shortly after the explosive argument at his Jupiter, FL mansion. Evelyn filed for divorce three days later.

Following the arrest, Chad pled “no contest” to the domestic violence charge and was sentenced to 12 months probation and domestic violence classes.

Chad was thrown back in the slammer last week after slapping his lawyer’s backside during a sentencing hearing.

He was released again yesterday following a probation violation sentencing hearing. During the hearing, the judge called the head gash photos “horrific.”

We reached out to Evelyn’s rep who had no comment on the photos. Calls to Chad’s camp weren’t returned.

Justin Bieber fled after allegedly driving his Ferrari into a photographer late Monday night on Sunset Boulevard — and now police are on the scene … TMZ has learned.

Justin was leaving the Laugh Factory around 11:45 PM in his infamous white Ferrari. His pal Lil Twist was there as a passenger, and several paps were on scene shooting the duo.

Once Justin got behind the wheel he motioned for some of the photogs to clear out of the way — but when he drove off he appeared to pin one of them between the Ferrari and a parked car.

Justin did not stop. It’s unclear if he even knew someone was claiming he had been hit.

The paparazzo (not a TMZ employee) was taken by ambulance to a nearby hospital for treatment. Witnesses were unclear what, if any, injury he suffered … but some suspect he hurt his leg.

Former NBA superstar Allen Iverson has returned his kids to their mother … and insists he was NEVER holding them hostage in the first place … at least that’s what A.I. claimed in court last week.

We broke the story … Iverson’s ex-wife filed court docs insisting Allen had all 5 of their kids holed up at an Atlanta hotel and refused to hand them over to their mother on the designated swap date.

But TMZ has learned … Allen appeared in Fulton County court in Georgia last Wednesday and told the judge his ex-wife had full access to their kids during their stay at the hotel … and therefore he wasn’t doing anything wrong. We’re told Tawanna also testified that she could have picked up the children from Allen but didn’t want to drive 45 minutes to his location.

After A.I.’s testimony, the judge decided the former Philadelphia 76ers star didn’t deserve to be thrown in jail — despite his ex-wife’s request before the hearing.  The kids — ranging from ages 3 to 16 — were returned to their mother following the hearing.

In fact, sources connected to the case tell TMZ … Allen and his ex have worked out some of their issues … and she has already allowed Allen to spend more private time with the kids.

So far, no comment from either side.

The famed freeloader known as King Kevi – who rose to sub-fame by hanging out at Justin Bieber‘s house and pretending to be the singer’s BFF — now completely regrets his life choices … claiming the Belieber nation has turned on him in a big way, and threatened his life.

20-year-old Kevin Pederson has filed a request for a restraining order against the singer, claiming Bieber tweeted out his cell number on June 13th … unleashing a 40,000,000-person Belieber army on his ass.

Kevin says Bieber tweeted out the number as an act of vengeance against reports that he and Kevin are friends — insisting the two NEVER met.

Kevin claims Bieber deleted the number minutes later, but it was too late — his cell number had already been released to a vicious bloodthirsty mob of teen girls … and these girls do NOT mess around.

Kevin says he has since received thousands of death threats and he genuinely fears for his safety — claiming it only takes ONE crazy Bieber fan to end his life.

Bottom line, Kevin wants Bieber to stop tweeting about him — and he also wants Bieber and his goons to stay 150 yards away from him at all times.

Bieber’s reps claim Justin never tweeted the number in the first place.

Fun fact: Kevin is the same guy who staged late night pics at Selena Gomez’s gate earlier this year to make it look like he was hooking up with her.
